Orthodontia

Orthodontia (braces) is an eligible expense for Healthcare and Limited Purpose FSAs. The IRS allows two options for orthodontia reimbursement: an Installment Option or a one-time Full Payment Option. (Click here for a printer-friendly summary).

Installment Option

The initial down payment is reimbursable as soon as the treatment begins. The balance may be claimed as payments are made over the length of treatment.

Required Documentation

You will need to submit a provider contract from your orthodontist with your claim. The contract must include:

  • The date of installation
  • Estimated insurance towards the treatment
  • Total cost of the treatment
  • Estimated length of the treatment

You may use the Benesyst Orthodontia Reimbursement Support Form as an alternative to the provider contract.

Full Payment Option

If you pay in full at the time of installation, your full out-of-pocket amount is eligible for reimbursement. This is typically a one-time payment.*

*If at a later date you incur additional costs that are not covered by insurance, you may submit another claim.

Required Documentation

You will need to provide your receipt, the Explanation of Benefits (EOB) or proof of payment along with the Benesyst claim form.

You may fax this information to Benesyst. Or, for quicker reimbursement, file your claim and upload documents through the Online Claim Wizard.
